Its main purpose is to act as a bodyguard, protecting your insides from the outside world. Your skin is also a reflection of your internal health. Changes on the outside may signal health problems on the inside.Throughout your life, your skin will change constantly, for better or worse. In fact, your skin will renew itself approximately once a month. Proper skin care is essential to maintaining the health and vitality of this protective organ.

Skin Composition

The skin is made up of layers. It consists of a thin outer layer (epidermis), a thicker middle layer (dermis), and the inner layer (subcutaneous tissue or hypodermis).

Epidermis: The Outer Layer of Skin

The outer layer of skin, the epidermis, is a translucent layer made of cells that function to protect us from the environment. The most superficial portion contains dead skin cells that are continually shed. The deepest portion contains basal cells that are responsible for skin renewal. Keratin, a protein made within the cells of the epidermis, protects the skin from harmful substances, such as chemical products and bacteria. The epidermis also contains cells that produce melanin, which gives skin its color.

The epidermis is responsible for the look and health of the skin and it holds a large amount of water. The younger the body, the more water there is in the skin. The capacity of the skin to retain water decreases with age, making the skin more vulnerable to dehydration.

The epidermis is responsible for the look and health of the skin and it holds a large amount of water. The younger the body, the more water there is in the skin. The capacity of the skin to retain water decreases with age, making the skin more vulnerable to dehydration and wrinkles.

Keratin: Keratin is the strongest protein in your skin. It also gives hair and nails their strength.

Dermis: The Middle Layer

Layers of the Dermis
The two layers of the dermis are the papillary and reticular layers.

The upper, papillary layer, contains a thin arrangement of collagen fibers.

The lower, reticular layer, is thicker and made of thick collagen fibers that are arranged parallel to the surface of the skin.

Specialized Dermal Cells
The dermis contains many specialized cells and structures.

The hair follicles are situated here with the erector pili muscle that attaches to each follicle.

Sebaceous (oil) glands and apocrine (scent) glands are associated with the follicle.

This layer also contains eccrine (sweat) glands, but they are not associated with hair follicles.

Blood vessels and nerves course through this layer. The nerves transmit sensations of pain, itch, and temperature.

There are also specialized nerve cells called Meissner’s and Vater-Pacini corpuscles that transmit the sensations of touch and pressure.

The dermis contains two types of fibers that lessen in supply with age: elastin, which gives skin its elasticity, and collagen, which provides strength. The dermis also contains blood and lymph vessels, hair follicles, sweat glands, and the sebaceous glands, which produce oil. Nerves in the dermis sense touch and pain.

Collagen: Collagen is the most abundant protein in the skin. It makes up 75% of your skin. This is also your “fountain of youth,” for it’s responsible for warding off wrinkles and fine lines. Over time, environmental factors and aging diminish your body’s ability to produce collagen.

Elastin: This protein is found together with collagen and is responsible for giving structure to your skin and organs. As with collagen, elastin is affected by time and the elements. Diminished levels of this protein cause your skin to wrinkle and sag.

What is my Skin Type?

You’ve heard all the commercials. You’ve seen the advertisements in magazines. Everywhere you turn, even on the Internet, companies are selling skin care products that promise to reduce aging lines and provide a younger appearance. All the information can be pretty overwhelming.

Confused? Follow these tips to understand which skin care products are right for your skin.

The most important thing to remember when researching over-the-counter skin care products is to trust yourself: No one knows your skin better than you. There are a lot of skin care products on the market and it’s easy to waste a lot of time and money trying to find the best solution. So take a minute to educate yourself before purchasing skin care products. Remember, this information serves as a guide only. Be sure to check with your dermatologist, internist, or family doctor if you have specific problems with your skin.

Assess Your Skin Before You Buy

Before you consider buying any skin care products, there are a few basic facts about your skin you must know. These include:

Your skin type. Is it oily, dry, normal, sensitive, or a combination?

Your skin complexion. Do you have fair skin that burns easily or light to medium skin that may burn? Or do you have a medium tone that usually tans or a darker complexion the only rarely burns? Or is your complexion so dark that you never burn?

Your skin concerns. Do you want preventative maintenance to avoid premature aging? Do you have a skin problem, such as persistent acne, age spots, melasma (better known as “the mask of pregnancy “– when women get the darkened areas on sun-exposed parts of their face) or rosacea. You may also have large pores, sun damage, facial wrinkles or fine lines that require special attention. Do you have eye puffiness or under eye bags that will require special care?

Your personal habits. Are you a smoker? Do you spend a lot of time in the sun? Do you take a daily vitamin? Do you consume a well-balanced diet? All these factors will affect how you should care for your skin.

With this information, you can wisely sort through skin care products to find the ones suited for your specific skin type. If you need help, ask a Professional Skin Therapist (Esthetician) at your local health spa or physician’s office for his or her recommendations.

An Esthetician is specially trained to assess your skin type and condition, answer your questions regarding any concerns you have with your skin as well as take into consideration your health history and personal habits when making choices for treatments and skin care products best suited for your needs.

Note: Skin cancer is not addressed in this article. It is advisable to see a dermatologist for a “skin and mole” check every couple of years, especially if you have a fair complexion or a family history of skin cancer.

Normal Skin

This is the skin we’d all love to have. Soft, plump and with small to medium sized pores. Normal skin has good elasticity and is smooth and firm to the touch.

Oily Skin

Just look at most any teenager going through hormonal changes and you can see oily skin. It’s the only time in my life that my own skin was overly oil.

Oily skin looks shiny and also has a sort of hardness to it that goes beyond the nice firmness of normal skin—a sort of thickness. Pores are larger and more visible and, due to the excess oil, there’s more of a tendency toward pimples, blackheads, whiteheads etc. with the oily complexion.

What It’s Like: Oily skin is the curse of a pimple-plagued teen, but this greasy, sometimes shiny skin type can become a blessing as you age because it keeps you younger-looking than your dry-skinned contemporaries.

Treating It Right: Wash it more than once a day — as many times as you feel the need. Skin care products with alcohol will make the skin less oily.

Combination Skin

Combination skin is like having a split-personality. One part may be dry while another is oily. The term “T-zone” refers to combination skin types where the forehead, nose and chin are oily but the cheeks and the area around the eyes is either normal or dry.

This will fit the majority of combination skin types except for a small percentage who may have symptoms such as a pimply forehead or cheeks that are dry where all else is normal. In some cases, the skin could be normal everywhere outside the T Zone and this would still be combination skin type.

What It’s Like: Some areas are oily — often the forehead, nose and chin — while others, such as your cheeks, are normal to dry.

Treating It Right: Wash the oily areas, the so-called T-zone, at least twice a day with a non-drying cleanser.

Sensitive Skin

A person with sensitive skin has a lot more trouble with environmental factors than the average person. Their skin burns more easily in the sun, they have more sensitivity to cosmetics (particularly if perfumed) and may experience rashes or a burning sensation due to overexposure to wind, sun, and extreme shifts in temperature.

Sensitive skin tends to be thin, delicate with fine pores. It flushes easily, is prone to broken capillaries, is frequently allergic and can be rashy. Asian skin tends to be more on the sensitive side as does Irish skin.

Rosacea-Prone Skin

Rosacea may begin as a tendency to flush or blush easily. In fact, when people with rosacea think about their pasts, they often remember flushing or blushing more easily than most. This tendency to flush or blush easily can occur over a long period of time and may gradually progress to a persistent redness, pimples and visible blood vessels in the center of the face that can eventually involve the cheeks, forehead, chin and nose. Other affected areas may be the neck, ears, chest and back.

Rosacea affects the eyes in approximately 50 percent of the people who have rosacea.

While most common on the face, one recent study indicated that some people who have rosacea do not have it on their face at all but rather on their back or elsewhere.

Classic Signs and Symptoms of Rosacea
Signs and symptoms vary from person to person and are often intermittent. If you or someone you know exhibits any of the following, it is best to consult a dermatologist. Rosacea can be treated and controlled if medical advice is sought in the early stages. When left un-treated, rosacea often gets worse and then becomes more difficult to treat.

Flushing of Face/Neck
Rosacea can cause redness, similar to that of a blush or sunburn. The flushing occurs when increased amounts of blood flow through vessels at a fast rate and the vessels expand to accommodate this flow. The redness may become more noticeable and persistent as the disease progresses.

Dry Facial Skin
Some people notice that their facial skin becomes increasingly dry.

Pimples
The pimples of rosacea, which often occur as the disease progresses, are
different from those of acne because blackheads and whiteheads
(known as comedones) rarely appear. Rather, people with rosacea have visible small blood vessels and their pimples—some containing pus—appear as small, red bumps.

Red Lines
Some with rosacea notice red lines, called telangiectasia (tell-an-jek-taze-yah), which appear when they flush. This is due to small blood vessels of the face becoming enlarged and showing through the skin. These red lines usually appear on the cheeks—especially when the overall redness diminishes.

Bumps on Nose
Nasal bumps, a condition called rhinophyma (rye-no-feye-muh), are an uncommon sign seen especially in untreated rosacea. Men are more likely to experience the small, knobby bumps on the nose, and as the number of bumps increases, the nose and cheeks may appear swollen.

Facial Burning, Stinging or Itching
A 2002 survey conducted by the National Rosacea Society indicated that some people who have rosacea suffer physical symptoms, such as facial burning, stinging or itching.

Collagen Induction Therapy is a procedure which stimulates the skin to produce new collagen, thereby reducing wrinkles and improving skin texture. The principle behind collagen channeling using the Collagen Induction Therapy treatment is simple. When it is applied to the skin, under topical numbing agents, a roller with very fine needles or a special needle is used to create many microscopic channels deep into the dermis of the skin, which stimulate your own body to produce new collagen. These channels also improve the penetration of vitamins A and C creams about 1000 times. Vitamins A and C stimulate skin renewal, thereby making the skin appear fresher and younger.

Collagen Induction Therapy can typically cause the following results in your skin:

• Soften fine lines
• Contribute towards skin thickening
• Reduce the appearance of acne scars and other depressed scars on the body
• Help improve stretch marks

Advantages of undergoing Collagen Induction Therapy are:

• The process stimulates physiological responses
• The treatment is not painful
• A very short healing period - you may be back in public in 24 hours.
• The treatment is cost effective
• It can be used on thin skin
• There is no permanent damage to the skin
• It can safely be repeated for better results
• It can be used on laser resurfaced skin
• It can be used on all areas of the face, neck and body

Collagen Induction Therapy can be performed using topical numbing agents for most areas of therapy. After numbing, a roller with micro-needles or a special needle is gently passed over the area to create channels into the deeper layers of the skin.

The channels created enable very good penetration of topical solution or cream treatments applied after the procedure. Subsequent application of appropriate skin care products improves the skin’s overall result.
How long is the procedure?

Treatment sessions range from 10 minutes to 1 hour, depending on the area being treated.

How many treatments are needed?

This will depend on the degree of the problem and the patient’s expectations. Sessions are often spaced 2 – 4 weeks apart.

Is Collagen Induction Therapy painful?

The procedure usually requires anesthetic cream in some areas. There is usually no pain after the procedure.

What body parts can you have it done on?

Collagen Induction Therapy may be done on any part of the face, neck or body.

What is the recovery or downtime after? What should I expect after treatment?

Immediately after the procedure the skin is usually turns pink to red and may have areas of pinpoint bleeding. Redness may be noted within the first couple of days and may be covered by make-up.

Swelling is common for the first few days and may last 7-10 days. At the end of 7 – 10 days, most of the reactions would have subsided.

Normal cleanser, moisturizer and sunscreen can be used immediately after the procedure.

High concentration Vitamin A and C or other topical cream treatments like the Obagi System should be applied immediately after the procedure when the skin’s penetration is greatest.

What changes can be realistically expected with Collagen Induction Therapy?

Collagen Induction Therapy will usually make the skin smoother, fresher and improve fine lines around the eyes, mouth and cheeks. Skin texture and brown marks will also improve in most cases.

How long does it take to see improvement in the skin with Collagen Induction Therapy?

Collagen remodeling in normal skin takes at least 90 days, so the effects of this treatment may not be fully appreciated for at least 3 months after the procedure. But the effects last for years in most people.

How is it possible to get the best results from a treatment?

Evidence suggests that the results from Collagen Induction Therapy may be further improved by combining this treatment with Intense Pulsed Light (IPL) therapy (Quantum SR IPL) and the Aurora PhotoFacial. When the skin is red from this treatment, pulsed light is also better absorbed into the skin, and can further improve brown marks, broken capillaries and fine lines.

How long do the effects of Collagen Induction Therapy last?

The new collagen produced by CIT lasts for a long time, unless the skin is further damaged by sun exposure, smoking or other external factors. As the skin ages some of the new collagen will also age. The duration of results vary from person to person but should last for some years.

How much does Collagen Induction Therapy Cost?

Pricing is solely dependent on areas being treated and the number of sessions required for the desired results. Consultation is mandatory before treatment can commence to ensure complete medical history and understanding of the procedure by the potential candidate.

As professionals, we are constantly striving to improve the quality and outcome of our procedures. One of the simplest ways to do so is to focus on effective skin preparation. Now our pretreatment protocols nearly always include the Clarisonic Skin Care Brush. We have found that sonic cleansing technology provides a tremendous advantage over surface cleansing. After cleansing with the Clarisonic, the skin is noticeably more receptive to professional procedures and treatments, as as as to the patient’s home care regimen.

The central benefit of sonic pretreatment cleansing is evidence of increased absorption. One of our nurses was the first to observe that topical cosmeceutical products appeared to deliver increased benefit when preceded by use of the Clarisonic.

Benefits:

6 times more makeup removal

Leaves skin feeling and looking younger and smoother

Cleanses so well that products absorb better

Gentle enough for twice daily use

Helps reduce oily areas, dry skin patches and blemishes

Helps reduce the appearance of visible pores

Reduces the appearance of fine lines and wrinkles

Testimonial from Users:

“Want proof that it cleans? I wash off my makeup first with (“name of brand”) soap, and the washcloth is dirty. Then I use Clarisonic and the head is gray—clearly the soap and scrub are not getting all the dirt, but Clarisonic is.” —Stephanie M., Seattle, WA

Steven F. told us, “I have adult acne, large pores and fine lines. I have been using my Clarisonic for three weeks and my skin looks great. What is amazing is that something so gentle to your skin can actually make dramatic improvements.”

“It deep cleans my pores, reduces the appearance of fine lines and makes my skin baby-soft. It has my gray-looking skin appearing healthier and smooth. Shaving is no longer drudgery; I can get a close shave.” – Steven F.

Tania B. in Redmond, WA, “I was curious to try the Clarisonic as I had read such excellent reviews about it. I was ‘in love’ straightaway! I have also noticed that I don’t need as much foundation coverage as I did before. My skin feels noticeably softer, smoother and appears more toned.”

Wei L. in New York, “I just bought the Clarisonic skin cleaning brush and have been using it for about 2 weeks. I’m thrilled with the device and already recommended it to all my friends. I’m also a freelance make-up artist and will be recommending this to my clients as well.”

“In the 20+ years I’ve been in the cosmetic industry, there is nothing that I have ever come across that I have been so astounded by than my Clarisonic brush.” — Martin K.

“My Clarisonic, what can I say? To be without it would be like being without a toothbrush! It is definitely a must have for anyone with skin, so, that means, everyone must have one! I mean, who wouldn’t want clearer, firmer, younger, healthier, radiant looking skin? It’s really a no-brainer.” — Martin K.

Anita R. in Tigard, OR told us, “I am now 48 and I can honestly say that my skin looks better than it did when I was in my early twenties. And it’s all because of Clarisonic!”

“Now I can see how much better my anti-aging skin care is working. I get compliments every day on my skin, and I seldom wear foundation now. I will never go without Clarisonic. It’s a vital part of my every day skin care routine.” — Anita R., Tigard, OR

“I notice that my moisturizing products work better, using less.…I really rave to everyone about how the Clarisonic works to keep my throat and neck looking smoother and ’taut-er’. I use mineral makeup and find the Clarisonic really gets the residue out of my skin so much better than some abrasive cleansers I’ve tried. All in all, a great product that does what it says it will do and then some. I wish I had had this 30 years ago!” — Carol V.

“By enhancing cell repair, the aging
process is slowed down and damage to
the skin is reduced.”

The Pro-Niacin molecule is the backbone of the NIA 24 skincare
product line manufactured by Niadyne, Inc. (Research
Triangle Park, N.C. and Tucson, Ariz.), and distributed exclusively
through Mentor Corporation (Santa Barbara, Calif.).

“I truly believe that the Pro-Niacin molecule is one of the best
breakthroughs we have seen in anti-aging skincare since retinoic
acid,” said Julie Salmon, M.D., a dermatologist and partner at
Southwest Skin Specialists in Phoenix and Scottsdale, Ariz. “This
molecule is the most active form of niacin. Niacin has fantastic
effects on the skin. It is really the energy vitamin. In this active
form, niacin fuels cell repair. Although our body has a natural
repair system in place, it is not always at its optimum. Niacin actually
helps to energize this cell repair system at the DNA level.”
Dr. Salmon noted that many competing skincare products focus
on collagen or surface changes in the skin. “But repairing DNA
is a direct key to potentially reducing the risk of skin cancer and
slowing the aging process.”

All four NIA 24 skincare products: a Gentle Cleansing Cream,
a Physical Cleansing Scrub, a Skin Strengthening Complex
repair cream and a Sun Damage Prevention 100% Mineral
Sunscreen SPF 30, contain the Pro-Niacin molecule.
Dr. Salmon strongly recommends the repair cream. “It is non-irritating
and very cosmetically elegant. It also provides outstanding
hydration by reducing transepidermal water loss.” Patients apply
the cream at night or twice a day. Another option is for patients
to use the NIA 24 sunscreen during the day and the repair cream
at night.

According to Dr. Salmon, the cleansing scrub provides “a nice
amount of gentle exfoliation with jojoba beads, which houses the
active ingredient. The beads provide gentle physical exfoliation,
so it is a great product to use in the shower, for instance.” People
can use the cleansing scrub on the face, neck, chest and even
hands and arms, two to three times a week.

For daily usage, the SPF 30 mineral sunscreen “is excellent.
For daily facial sunscreens, once the SPF is above 30, you tend
to compromise the cosmetic elegance, which decreases patient
compliance,” Dr. Salmon noted. Mineral sunscreens are less irritating
than chemical sunscreens and offer broad-spectrum protection.
The mineral sunscreen can also act as adjunct protective
therapy after a light chemical peel or a laser procedure for
optimum full broad-spectrum ultraviolet A and B coverage.
“Mineral sunscreens reduce the potential for irritation,” Dr.
Salmon pointed out.

Typically, within the first month of using a combination of the
NIA 24 skincare products, “patients will notice that their skin is
much smoother,” Dr. Salmon observed. “It is usually the textural
changes that people will notice first. Skin is also better hydrated.”
At about three months, “skin tone has improved considerably. In
addition, the general redness of the skin tends to be diminished
and much more even in its pigmentation. Brown pigment often
becomes softened.”

Dr. Salmon recommends that patients begin a skincare regimen
with the NIA 24 line because it is extremely non-irritating and
enhances hydration. “The skin barrier also becomes stronger and
patients notice immediate results,” she said. Later, patients can
comfortably start using a retinoid because “their skin has been
prepared to receive more irritating agents.”

Tess Mauricio, M.D., a dermatologist and owner of Scripps
Ranch Dermatology and Cosmetic Center in San Diego, Calif.,
has been dispensing the niacin-based product line for about five years. “I think there is very good science behind it, so it is not your
ordinary cosmeceutical,” she noted. “Patients are able to tolerate
the products well and they are efficacious. Patients are very, very
happy with the results. I recall only two people over the past five
years who were unable to tolerate the products because of slight
irritation. Every other person has told me they love the products.”
Besides being easy to apply, the NIA 24 product line acts as
an anti-inflammatory agent, “so it is great for patients with chronic
sun damage or those who have very sensitive skin from
rosacea, for example,” Dr. Mauricio said.
Furthermore, the Pro-Niacin molecule “has good science to show that it prevents the
transfer of pigment to the top layer of the skin, so it sort of acts as
a lightening agent, but without being a hydroquinone product.”

Hence, the NIA 24 line is “great for maintenance, especially for
patients who undergo laser treatments. There is also a decrease in
the recurrence of brown spots, broken capillaries and rosacea.”
Patients may also reap the medical benefit of potentially reducing
sun damage.

Dr. Mauricio encourages most of her patients to invest in all four
NIA 24 products if they are financially able. “I love every single
one of these products.” The complete package costs the patient a
little more than $200 for a two to three month supply. “This is not
very expensive compared to other physician-dispensed products,”
Dr. Mauricio noted.

The Physical Cleansing Scrub should be used two to three times
a week to exfoliate skin, while the Gentle Cleansing Cream should
be used to cleanse on days when the Physical Cleansing Scrub is
not used. The Skin Strengthening Complex repair cream should be
applied twice a day, and the Sun Damage Prevention 100%
Mineral Sunscreen SPF 30 in the morning, every day.
“Given that the Pro-Niacin molecule can reverse some sun damage
is a big plus for a lot of my patients here in San Diego
because many have been sun worshippers for years,” Dr.
Mauricio said.

In a post-Fraxel laser study, “we found that every single patient
could tolerate treatment when NIA 24 was applied afterwards,”
Dr. Mauricio conveyed. “NIA 24 products also accelerated healing
and minimized redness.” Healing time was reduced from
around five days to three days.
According to Dr. Salmon, “The Pro-Niacin molecule addresses
top to bottom care for the skin. In the epidermis, water loss is
reduced. The molecule also helps thicken the epidermis. And in
the dermis, the molecule helps the health of the collagen by
reducing some of the glycation and hardening of the collagen,
which occurs with age and contributes to wrinkles. Nearly all my
patients love the NIA 24 products. Skin looks better because it is
healthier.”

Hi there,

I wanted to order some more products but I needed some clarification first.

Q. I’m currently finishing off my Neostrata Plus AHA 15% for the face and I’m looking to pick up a new AHA, so what is the difference between DCL AHA revitalizing lotion for the face 10% or 20% and the MD Forte Facial Lotion 15%? There seems to be a large price difference, is the pH level relatively the same, are they equally good manufacturers of AHA?

A. DCL AHA Revitalizing Lotion for Face 10% is a 10% glycolic acid based lotion whereas the former is a 20% glycolic acid lotion (twice the exfoliating power). If you feel your skin needs a “bump” up in exfoliation – then I would recommended going to the 20% strength not backward to the 10%. The difference between DCL and MD Forte is the way the glycolic acid in the products are buffered. DCL can cause redness and irritation on the skin and in some cases cannot be tolerated by more compromised skin types (such as sensitive skin or rosacea prone skin). They both contain the active ingredient “Glycolic Acid” but MD Forte is not as harsh on the skin so many customer’s like this product because it causes no skin reaction and they still get clarity and smoothness to the skin. MD Forte is also considered more “cosmetically elegant” meaning the products are formulated with base ingredients that, when applied, glide easily across the skin and absorb quickly and provide increased moisturization in the process.

Q. Secondly, is it important to use a cleanser with an AHA when using glycolic creams? A Neostrata rep told me that Neostrata cleansers, Cliniderm cleansers and Spectro Jel cleansers don’t strip the skin, so they’re ideal when using AHA products on the face. Is the MD Forte cleanser 12% too strong, if I’ve only been using the Neostrata 4% cleanser?

A. When choosing a cleanser to use with an AHA program, it is important to realize that the cleanser must not only remove dirt, makeup and oil but must also prepare the skin for the next step in the treatment program. I always recommend that you use the cleanser in the same line of exfoliating products you choose to use. Why? Because the cleanser has been formulated to work synergistically with those treatment products and adequately prepares the skin so the treatment products work as effectively as possible. Usually you will choose the cleanser in the line based on your skin type as most manufacturers offer more than one cleanser. The percentage of glycolic acid in the cleanser basically only prepares the skin, since a cleanser is washed off – the glycolic acid and any other active ingredients in the cleanser do not stay on the skin for long enough to actually exfoliate, but they do reduce the pH of the skin to the point where the treatment product in the line works most effectively.

Q. Thirdly, does it make a difference using a Vit. C serum verus a a Vit. C cream? When is the best time to use my Vit. C if I’m using a AHA in the morning and my retinol at night? Can I layer the serum underneath my AHA? ( I also use an SPF 60 by Ombrelle or La Roche Posay daily on top of my AHA)

A. You choose the Vitamin C based on your skin type or the skin care regimen you are using. Meaning, if you are using three or four products in the morning and you want to add another treatment product in that program, then you need to evaluate the base of the products you are currently using to determine which base you choose in the Vitamin C. Vitamin C should be used in the a.m. (as the product is designed to protect the skin from free radical damage which comes from UV exposure). Proper layering of products is always thinnest to thickest. So, in your case, say your using an AHA Cream in the morning (because your skin is on the normal/dry side) and you want to add a Vitamin C – you would choose a Vitamin C Serum. Why? Because if you chose the Vitamin C Cream and you are using an AHA Cream – both are thicker bases so the Vitamin C Cream base will not allow the AHA Cream base to penetrate the skin and do it’s job. Serums are almost always a safer bet when layering products. You would use the Vitamin C Serum (after cleansing) let that absorb into the skin, then apply your AHA Lotion or Cream over that, and then lastly, your sunblock. Continue to use your retinol at night after cleansing.

Q. I’ve been using a Vit. A acid (tretinoic acid?) from my dermatologist in a 0.025 or 0.05% nightly for a few years. Would the Skinceuticals Retinol 0.5% night cream, or the Retinol Complex, or the Night Complex Level I be a better alternative? Are the roughly the same thing? Also, can I apply the retinol sparingly around the eye area?

A. If you are already using a prescription strength retinol (tretinoin) from your physician you would get little benefit from changing to SkinCeuticals Retinol. They are not comparable. Prescription strength Retin-A is much more effective and always a better choice if you can tolerate it. Most people go to the alternative (non-prescription) strengths because they either do not want to get a rx for a retin-a from their physician or they simply cannot tolerate prescription strength retin-a. Whether or not you can tolerate your Retinol around the eye area remains to be seen. The only way to know is to try it once and see how the skin of the eye area tolerates it. I would recommend you either start with the lower strength retinol to see how you react to it or mix the retinol with a moisturizer to dilute the potency for less irritation. You may want to touch base with the dermatologist that prescribed the product to see what he/she recommends before trying it.

I hope this information has been helpful to making your product selections. Please feel free to contact me at any time if you have further questions – I am happy to share my knowledge with you and hopefully, the selections you make will pay off with smooth, beautiful skin (which is what we ultimately strive for). I have used just about every product line out there at one time or another and I am currently embarking on testing out the Obagi Skin System to see how this system differs (in results) to other lines I have used in the past. Up to this point, I have not felt the need to try this system (which is pretty aggressive) but as I get older I see changes in my own skin that warrant changes in my skin care routine. So, if your ever interested in feedback on this system, I will be able to offer my experience with this soon!

Radiant Skin Clinic always strives to bring you up-to-date information on products available through physicians and offer you knowledge and understanding of skin care in general. Please feel free to contact our estheticians ANY TIME with questions or concerns you may have. We promise to give you sound advice and recommendations - our goal is not to “sell you products” but to help you discover the best skin care for your needs.

VIVITE is the newest skincare introduction by Allergan Medical, makers of physician-only skincare treatments MD Forte, Prevage MD and the anti-aging prescription Avage as well as Botox. VIVITE skin care exfoliates, hydrates, and protects.

Exfoliation is important in removing non-productive cells from the skin’s outer layer to reveal healthy, new skin. For exfoliation, VIVITE contains powerful glycolic compound, as well as exfoliating microbeads.

Hydration is important in keeping skin smooth. For hydration, VIVITE contains effective moisturizers.

Antioxidant protection is important in keeping skin from aging prematurely due to attack from free radicals present in environmental stressors. For antioxidant protection, VIVITE skin care contains botanicals that stop free radicals from attacking your skin.

What products are in the VIVITE routine?

VIVITE Exfoliating Cleanser dissolves skin build-up with both chemical and mechanical exfoliation and contains exfoliating microbeads to more completely remove unproductive skin and allows new skin to flourish. VIVITE Exfoliating Facial Cleanser loosens the unproductive top layer of skin so you can scrub it away and reveal fresh, new skin. Contains 15% glycolic compound. To be used 2 to 3 times a week.

VIVITE Daily Antioxidant Facial Serum moisturizes and extends exfoliation to reduce the appearance of fine lines and wrinkles as well as provides antioxidant protection. VIVITE Daily Antioxidant Facial Serum is a regular application of glycolics for hydration and renovation of the top skin layer. Also provides antioxidant protection to fight premature aging. Contains 15% glycolic compound as well as green tea, pomegranate and licorice — all noted for their natural antioxidant properties — as well as chamomile and aloe to smooth and soften skin. To be used once daily, every morning. Perfect under makeup.

VIVITE Night Renewal Cream is an extra-rich formulation that promotes overnight maintenance and repair of skin tissues. Contains 20% glycolic compound as well as Vitamin C and Vitamine E. Also contains superoxide dismutase (SOD), an advanced antioxidant for effective topical protection against scavenging free radicals. To be used once daily, in the evening.

VIVITE Revitalizing Eye Cream is specially formulated to reduce the appearance of fine lines, wrinkles, and dark circles in the sensitive areas around the eye, as well as minimize the appearance of dark circles. Contains 10% glycolic compound in addition to caffeine to reduce puffiness and inflammation, peptides to tighten skin and interference pearls that reflect and diffuse light, improving the appearance of fine lines and wrinkles, as well as dark circles under the eyes. To be used twice daily, morning and evening.

Note: Products can also be used individually. Use an SPF 15 or greater sunscreen to prevent an increase in sun sensitivity.

VIVITE combines 10% to 20% glycolic concentrations with antioxidants to promote moisture and stimulate collagen production. Glycolic acids, also known as one of the alpha hydroxy acids or AHAs, have been shown to improve the look and feel of the skin on your face and body. AHAs have been the most successful ingredients in the history of cosmetics, and have been used since the days of Cleopatra. Glycolic acid is believed to be the most effective AHA because of its small molecular size for greater skin penetration. It is partially neutralized so irritation is minimized. For more information on glycolic acid, please read our resource article under the SKIN TALK link “Exfoliation Restores a Youthful Appearance.” Glycolic acids are also found in the MD Forte skincare product line (the every day glycolic routine). Radiant Skin Clinic has been recommending the MD Forte skincare regime for over 7 years now and we have seen incredible changes in our patient’s skin after just a few short weeks. We are very excited about offering this new product line from the makers of MD Forte.

The VIVITE skin care routine is also rich in botanicals including olive leaf extract, pomegranate, and green tea for natural antioxidant protection; aloe, chamomile, and wild mango butter for hydration; and licorice to reduce hyperpigmentation. The ingredient list also includes antioxidant vitamins C and E, as well as caffeine to reduce puffiness and inflammation. There are advanced ingredients in the VIVITE skin care formulations: superoxide dismutase (SOD) to provide antioxidant protection and neutralize free radicals, peptides to tighten skin, microbeads to enhance exfoliation, and interference pearls to reflect and diffuse light as well as minimize the appearance of dark circles under the eyes.

While the glycolic strength is high, the antioxidants are gentle — even sensitive skin types will benefit. The most visible effects of Vivité is improved hydration and smoother skin texture, all without harsh chemicals. It is non-greasy and absorbs well and smells great.

Clinical Study

A clinical study of the VIVITE skin care routine was conducted with 22 women between ages 35 and 55 showing the effects of mild to moderate photodamage (rough, dry facial skin; fine lines; and wrinkles). They followed the study regimen for 3 weeks. The results after just 3 weeks:
•	22% reduction in fine lines and wrinkles
•	36% increase in skin hydration/moisture content
•	54% improvement in skin smoothness
•	65% decrease in dryness

HOW does it work?

By stimulating skin’s natural barrier function, NIA 24 builds stronger, better skin that’s more resilient and resistant to damage. By improving the skin’s barrier function, overall skin tone and texture improve. NIA 24 reduces the appearance of visible brown spots, hyperpigmentation and lines specifically associated with sun damage. Without it, skin falls prey to daily sun and environmental stresses that slow skin’s self-protective function and speed the visible signs of aging.

This special form of Pro-Niacin delivers immediate and residual benefits, as well as continuous dermal-delivery of Niacin without vasodilation and photosensitivity.

WHY does it work?

During research partially funded by the National Institutes of Health, scientists identified an important role for the active form of Niacin in how cells respond to genetic damage. NIA 24 introduces that breakthrough: patented Pro-Niacin.

While Niacin is used in other products on the market, Pro-Niacin is the first and only patented Niacin molecule that converts its active form to regenerate skin cell layers.

Improvements in skin texture, smoothness and appearance of hyperpigmentation in a subject treated with Pro-Niacin:

Who’s NIA 24 for?

NIA 24 is ideal for any patient, both female or male, who sees-or doesn’t want to see-the premature signs of aging, from lines to age spots. As a skin strengthener, it’s also a necessary companion for those undergoing serious skin care or procedures.

What you may experience when first beginning the NIA24 Skin System:

First Blush:
*	A warming sensation that can lead to redness or irritation. Follow an introductory regimen and apply a cool compress to soothe skin.

Initial Breakout:
*	As skin adjusts to increased Niacin, a breakout may occur due to temporarily increased sebaceous activity. Follow the recommended introductory regimen.

Dryness:
*	Depending on environmental and dermal moisture levels, skin can experience increased dryness. The barrier’s ability to retain moisture and resist damage inside can make surface skin feel dry.

Reductions in rosacea-like redness and evening out of skin tone:

Skin’s better outside.

In clinical studies, patients see dramatic differences in just 90 days:
90% reduction in hyperpigmentation
82% improved skin texture
81% increased skin smoothness
80% improved skin tone
68% reduction in fine lines & wrinkles
53% improved firmness

Improvement in broken capillaries and irregular skin tone from sun damage:

How are NIA 24 products different?

NIA 24 products are the only comprehensive, science-based protocol to take every action - preventative, pro-active and corrective. Containing Pro-Niacin – a patented, continuous-release system of Niacin, NIA 24 products were born from science and developed with funding from the National Institutes of Health and clinically researched at the University of Arizona Cancer Center. NIA 24 technology is so advanced, it holds 14 patents.

Dermatologist-approved PCA SKIN Clinical Care Products are scientifically formulated using biocompatible and chirally correct ingredients such as alpha and beta hydroxy acids, peptides, antioxidants and advanced botanicals. PCA home care products will nourish, hydrate and rejuvenate your skin.

Free of color additives, synthetic fragrances, harsh alcohols, comedogenic oils and other known sensitizers, PCA home care products are designed to deliver results without irritation.

We recommend using medically dispensed PCA home care products in addition to your clinical treatment plan to further enhance your results.

Whether battling acne, rosacea, sun damage or aging skin, daily use of PCA SKIN Clinical Care Products will dramatically improve skin health and help provide the glowing, radiant complexion you desire.

Many of the PCA SKIN® Clinical Care Products contain a blend of lactic and citric acids which promotes healthy cell turnover and allows the products to remain pH balanced.

Alpha Hydroxy Acids (AHAs):
All alpha hydroxy acids have the effect of dissolving the bonds between skin cells, to allow for exfoliation that will bring newer cells to the surface. Each AHA also has additional benefits:
Lactic Acid – hydrating and helps even skin discoloration
Malic Acid – hydration
Glycolic Acid – fast penetration [which can be a negative] and oil control
Citric Acid – prevents and corrects the thinning of the skin associated with aging and aids in hydration
Tartartic Acid – antioxidant action from grapes
Mandelic Acid – possible effect on evening skin discoloration

Antioxidants
Antioxidant – A group of vitamins, minerals and enzymes that help repair and prevent cell damage caused by free radicals. Free radicals are responsible for premature aging by damaging cells and breaking down collagen and other proteins within the skin.
Green Tea - Tea is an excellent source of antioxidant polyphenols and anti-inflammatory agents. EGCG (Epigallocatechin Gallate), which is a component of green tea, is considered the most powerful polyphenol of them all. It offers skin soothing benefits, protects the skin from UV induced damage and is being studied very carefully for possible anti-tumor and anti-cancer capabilities.
Tocotrienols and Tocopherol (Vitamin E) - these forms of vitamin E have superior antioxidant benefits due to their exceptional ability to penetrate the skin cells.
Grapes –The flesh, skin and seeds of the grape all contain polyphenols that offer many skin health benefits.
Olives – a great source of antioxidant polyphenols and vitamin E.
Retinol (Vitamin A) - has strong antioxidant properties along with the ability to increase cell turnover for healthier and brighter looking skin.
Soy – the isoflavones found in soy are potent members of the polyphenol family. They work to plump and hydrate the skin. They fight aging and wrinkle formation by building the collagen matrix. They also effectively quench the free radical damage that speeds the aging process.
Cherries - help to fight free radicals and acts as potent anti-inflammatory ingredients.
Citrus Fruits – are a great source of polyphenols. A number of citrus fruits not only have antioxidant value but also increase the amount of vitamin C within the cells.

Examples of PCA SKIN® Clinical Care Products containing antioxidants:

Pigment Control
Hydroquinone -the only ingredient currently approved by the FDA as a skin lightener. HQ works primarily by bleaching existing skin discoloration and also prevents future pigment from occurring.
Kojic Acid – an antibacterial and antifungal agent that evens skin tone.
Licorice – a very effective ingredient for gently evening skin discolorations that also has antibacterial and anti-inflammatory benefits.
Lactic Acid – (sour milk) evens skin discoloration, hydrates and reduces the build up of dead skin cells.
Ascorbic Acid - (primarily citrus fruits) acts as a natural skin brightener.

Peptides
Peptides are synthesized protein fragments designed to perform specific functions in the skin and body.

Here are a few peptides of note:
Acetyl Hexapeptide-3 (Argireline) - works by relaxing facial muscles that cause repeated facial expression.
Dipeptide-2 - improves lymphatic circulation to more effectively remove toxins in the skin. Improved lymphatic flow will help to minimize dark circles and puffiness by having fresher healthier looking skin and less pooling of fluid and blood under the eyes.
Palmitoyl Tetrapeptide-3 -increases skin firmness and elasticity while diminishing eye area puffiness
Palmitoyl Oligopeptide – Palmitoyl Tetrapatide-3 (Matrixyl 3000™) a blend of two advanced peptides that provide visible anti-wrinkle activity.
